Performance Rehab will provide the highest level of quality rehabilitation services in physical therapy and occupational therapy. We will maintain excellent standards in customer service, physician communication and flexibility to meet both the needs of our patients and referral sources.

Performance Rehab provides physical therapy services at our Oelwein clinic and provides both physical and occupational therapy outpatient and inpatient services at Community Memorial Hospital in Sumner, Iowa. We also provide rehabilitation and consultative services for business industry and home health clients throughout northeast Iowa.
